Additionally , as contrary as it may seem, it could actually be the desire to protect the subject in the piece. they're left helpless and vulnerable and the viewer wants to protect them and make them their own. While I can't speak for all of them I'm sure many guro enthusiasts don't imagine themselves inflicting this sort of harm on the subject.

First of all is the artistic talent that goes into such a piece, incredible amounts of detail drawing organs, muscles, body parts, with careful anatomic precision puts pieces like this on the top shelf of artists collections.
Secondly, as a gore function, there is something soothing in the theme of romantic gore, the concept that you could trust someone enough to physically harm you without killing you is the closest two individuals can get.
As it gets more violent, guro has to do with the softness and warmth of blood and organs, skin, and also in a romantic sense, the ability to literally be inside somebody and become closer than anyone else.
Guro also uses many forms of dominance, similar to popular rape-culture themes works you see in Anime these days. Taking out ones aggression though violence is more common than through sex, and a lot of people who read or view violent and rape hentai or pornography often wonder as to why the victim is never killed. Certainly you would not let a girl who is the lead character from an action anime live to get her revenge after you raped her would you?
Also, there is the aspect of dominance, the assertion of power and strength.
Finally, I would like to point out the culture of modern metal music, which often deals with themes of mutilation, gore, and violence. Guro art is not very different from the imagery and themes used in the albums of artists like Slipknot, As I Lay Dying, Job for a Cowboy, Dimmu Borgir, Iron Maiden, Chelsea Grin, Dillinger Escape Plan, Cattle Decapitation, and Dying Fetus, all of which are highly popular bands.
So what makes this style of art any normatively "worse" or more disgusting than violent video games and music?
