I suppose that there is no reason that the futanari pseudo-penis has to be connected to the urethra, I suggest that the 3 holes connect in top to bottom order: male gonad(s)[pseudo-penis], bladder {urethra], female gonad(s)[vagina].
Or in a more interesting theoretical biology the futanari's member is actually an ovipositor for implanting her fertilized eggs in a host.
